In addition, our Care Professionals and are fully screened, trained, insured and bonded and supervised by a registered nurse. We provide ongoing training designed to expertly train, engage, and retain the best care providers and companions. Our extensive training exceeds our state’s requirements, and harnesses our 24 years of industry expertise, to ensure our care staff are knowledgeable in best practices and fully equipped to assist clients with diverse care needs. Additionally, each member of our team receives specialized Alzheimer’s and dementia care training through the Alzheimer’s Association.

FIRST CONTACT

Upon receiving an inquiry regarding care services, the owner and registered nurse will come to your home on an appointment basis. The appointment is set at a time convenient for you and your family.

INITIAL MEETING

During this initial meeting, an assessment is performed by our registered nurse to identify your needs, requirements, and overall home-situation. At this time we will explain our in-home care services and suggest the level of service deemed most appropriate. The initial meeting leads to a mutually acceptable Plan of Care.

PLAN OF CARE

The Plan of Care is a document outlining the specific care services, schedules, client preferences, and special instructions, which At Home Eldercare employs to achieve your satisfaction. As your situation changes, the Care Plan is modified to reflect those needs. It provides a central focus to all parties involved with your care. It’s the roadmap by which the Care Professional will provide you care on a daily basis.

INTRODUCING THE CARE PRO

Once a service agreement is completed, we make every effort to match you with the right Care Professional based upon personalities, skills, expertise, interests and attitudes. Our guarantee is that we will find exactly the right Care Professional for you and your family!

CONTINUED FOLLOW-UP

After the Care Professional has been placed in your home, the RN and owner will continue to make periodic visits and telephone check-ins to you and your assigned Care Professionals to ensure the best possible service is being provided.
